It is believed that there are about 9 openings in the nipple surface, each of which are connected to ducts, which are connected to ductules and then alveolus’ (where milk is stored until let-down).
When let-down happens, the alveolus contracts and milk is ejected into the ducts and out the nipple openings. For more information on let-downs and how to speed up or slow down your let downs, see my blog post here.
The various ductal openings can point in any direction. They don’t just point forwards. You really see this when you hand-express your milk. Your milk can go in literally any direction!!!
